MSHSAA Physical Medical Eligibility
All student athletes must have a Medical Eligibility form on file and an Annual Requirements form on file. The Medical Eligibility form is valid for up to two (2) years. The Annual Requirements form is valid for one (1) year.
Both forms are uploaded to Healthy Roster by parents/guardians. Information on how to access Healthy Roster is listed below:
